Wellness Hub LLC (“we,” “us,” or “our”) operates https://www.wellnesshubutah.com/ (the “Site”). This policy explains what information we collect through the Site, how we use it, and the choices you have. It applies only to activity on this Site.
This policy is not our Notice of Privacy Practices. Information you share with us as a client or patient in the course of care is governed separately by our Notice of Privacy Practices and applicable health-privacy law, not by this policy.
Information we collect
Information you choose to provide. If you use our contact form, we collect what you enter: your first and last name, email address, phone number, insurance information (optional), and any message you write. You control what you type into these fields.
Please note: The contact form is hosted by SimplePractice, our practice management system, and submissions go directly to it over an encrypted connection. Even so, please share only what you need to in an initial enquiry. If you go on to become a client, we will gather your full history through our secure intake process.
Information collected automatically. We set no cookies of our own anywhere on this Site, and we use no Google Analytics, no advertising pixels, and no tracker that follows you across other websites. We do not build profiles of visitors. The only measurement we use is Cloudflare Web Analytics, which counts page views without setting any cookie and without storing an identifier for you. Our web host may also keep standard server logs for security and to operate the Site, but we do not use that information to identify individual visitors. The contact page is the exception. Our contact form is provided by SimplePractice, and its widget loads services belonging to SimplePractice and its suppliers — including Stripe, its payment provider — when you open Contact Us. At least one of these sets a cookie, and they operate under their own privacy practices rather than ours. Every other page on this Site remains free of cookies and third-party services.
Our contact form
Our contact form is provided by SimplePractice, the practice management system we use to run the practice. Selecting the contact button opens SimplePractice's own secure form. What you enter goes directly from your browser to SimplePractice — it is never sent to, stored on, or seen by this website — and we read and respond to it inside SimplePractice.
SimplePractice is a healthcare platform that operates under a business associate agreement with the practice, which is the arrangement HIPAA requires of a service handling protected health information on a provider's behalf. The Site itself is hosted by Cloudflare, which serves our pages and provides security and performance for the Site. Because contact form submissions never pass through this website, Cloudflare does not receive them. You can review Cloudflare's privacy practices at https://www.cloudflare.com/privacypolicy/.
Cookies
We set no cookies of our own, anywhere on this Site — not for functionality, analytics, profiling, or advertising. Cloudflare Web Analytics is cookie-free by design. The contact page is the exception. The SimplePractice contact widget loads third-party services, including Stripe, and at least one cookie is set when that page loads. Those cookies are not ours and are governed by SimplePractice's and its suppliers' own privacy practices. If you would rather avoid them entirely, you can reach us by telephone on 801-210-0146 instead.
How we use information
We use the information you submit through the contact form to:
- respond to your inquiry;
- operate, maintain, and secure the Site; and
- meet legal, security, and record-keeping obligations.
We do not sell your personal information, we do not use it for targeted advertising, and we do not use it to send marketing messages unless you separately ask us to.
How we share information
We share information only as needed to run the Site and respond to you, including with:
- Service providers that help us operate the Site and the practice — our website host and analytics provider (Cloudflare), and our practice management system, which hosts the contact form (SimplePractice). These providers process information on our behalf and are expected to protect it.
- Legal and safety recipients, when we believe disclosure is required by law or necessary to protect the rights, safety, or property of any person.
We do not otherwise share the information you provide with third parties for their own purposes.
Data security
We use reasonable administrative, technical, and physical safeguards to protect information collected through the Site, including serving the Site over an encrypted (HTTPS) connection. Contact form submissions are transmitted directly from your browser to SimplePractice over an encrypted connection and are stored there, not on this website. No method of transmission or storage over the internet is completely secure.
Data retention
We keep contact form submissions, which are stored in SimplePractice, only as long as needed to respond to your enquiry and to meet our legal, professional, and record-keeping obligations.
Your choices and rights
You can contact us at 801-210-0146 to ask about the information we hold from your contact-form submission or to request that we delete it.
Depending on your state of residence, you may have additional rights regarding your personal information. Utah residents can learn more about their rights under the Utah Consumer Privacy Act through the Utah Division of Consumer Protection. To exercise any available right, contact us at 801-210-0146.
Children’s privacy
The Site is intended for adults. We do not knowingly collect personal information from children under 13 through the Site. If you believe a child has provided information through the Site, please contact us and we will delete it.

The Site may link to third-party websites we do not control. This policy does not apply to those sites, and we encourage you to review their privacy policies.
Changes to this policy
We may update this policy from time to time. When we do, we will revise the effective date above and post the updated policy on this page.
